Movie Monday - April 4 - Qantas Flight 32 in focus By
Jon Ostrower  on April 4, 2011 10:43 AM | Permalink | Comments (5) | TrackBacks (0) | This week's Movie Monday comes from the ABC (Australian Broadcasting Corporation), not to be confused with the American Broadcast Company. Four Corners devoted a full episode to examining the A380's uncontained Trent 900 engine failure that befell Qantas Flight 32 on November 4 of last year. I'll confess that I have not had a chance to watch the entire program yet, but what I have seen presents it as the most comprehensive look at what happened to QF32, re-created with the full participation of the airline. 
The 45 minute look at QF32 also includes an additional 45 minutes of extended interviews with Captain Richard De Crespigny, Qantas CEO Alan Joyce and Alan Milne, head of maintenance operations.

Jon,

Thanks for posting I was surprised how good a job a reporting job four corners did with this. As a 30 plus year industry veteran it is unusual that I can watch something like this and not find fault. I have some personal experience with a CF-6 un-contained failure. I remember my surprise at looking at a #2 engine on a DC-10 and realizing that everything aft of the The #1 HPT was essentially gone. Modern airplanes and engines are becoming incredibly complex machines and I'm constantly amazed how reliable they are given that complexity.

Great report. I'm sort of surprised that a billion dollar plus R&D program cannot catch an eccentric drill in a pipe part during the manufacturing process, given the fastidiousness for tolerances in aircraft engines. Anyhow, I am assuming the software for the CNC drilling process will now include a warning like "the axis of the drill does not match the axis of the tube. Are you sure you want to go through with it?"

For me, as an ex-Flight engineer , it was almost an eye-wittness story hearing the interview of the captain.

Does the documentary or other reporting answer the question, why has this happened only to Qantas. Their overall recent safety record seems quite bad, including the uncontained failure of 744 engine around the time of this A380 failoure.
